I hereby give my consent to and authorize my minor child to participate in all activities of the Jubilee Summer Program. On behalf of myself, my child, and my child's personal representatives, assigns, heirs, distributees, guardians, and next of kin ("Releasers"), I hereby irrevocably and unconditionally release, waive, discharge, and covenant not to sue Jubilee Christian School, Faith Church and any affiliates and all teachers, administrators, pastors, ministers, leaders, employees, volunteers, representatives and agents (collectively "Jubilee"), for and from all claims of any nature now or hereafter existing whether known or unknown, including but not limited to, all liability to the Releasers, on account of injury to or the illness of my child or injury to my child's property, whether caused by the negligence of Jubilee or otherwise, arising out of or in connection with all activities related to the Summer Program, or any travel connected therewith.
I further agree to indemnify and hold harmless Jubilee, from and against any claim, demand, action, loss, debt, expense, or damage (including reasonable attorney fees) which may arise by or result from my child's participation in all activities related to the Summer Program, including, but not limited to, those involving transportation, medical and/or dental services.
